First of all, I purchased both the extra firm and standard density of this same brand of pillow. All pillows are of superior quality. However, I would say the standard density really is firm, and the extra firm is "extra extra" firm. The exterior of the pillow is nice, soft, and of excellent quality. All pillows have nice, dense filling, without being too hard. However, the thickness is a bit of a problem for me for the extra firm, and I like my pillows to be very firm. If you like a firm pillow, but don't want to risk having a sore neck from the pillow being too thick, I would just go with the firm or even the standard instead of the extra firm. I bought the standard density for my kids, and even those are quite thick and too big for their little heads (but perfect for me). I think the extra firm is too thick for smaller people (I'm 5'3"), but for larger people like my husband (who is 5'11"), works well.
