Finally...
I have tried EVERY...and I mean every...heel insert - gel type, ones like sandpaper, Petals, Dr. Scholls...all of them. This is the only one that has worked for me. I think it's because it's not a uniform size. Get that raised strip to hit the right spot on the back of your ankle and it stops the heel from slipping out of the shoe. The cover material is a rough nu-buck type - soft and it adds a little grip. One warning - the adhesive on these is strong. If you need to remove these don't take off all the backing. I usually expose just about an inch in the center and that keeps these on most shoes.
