Size matters.
I'm a short woman with a short neck. I usually fall asleep on my side and then roll (while sleeping) onto my back. The small sized pillow is almost perfect for me. The foam's not hard enough to hurt my ear or give me a headache, but also not so soft that my head sinks straight down to the mattress. It's very supportive and keeps my neck properly aligned. I wake up with no neck pain or stiffness. The small pillow is noticeably smaller in length, width, and height than the other sizes (except the travel size, of course). I also have the medium pillow but couldn't sleep well on it because it's too high for my neck. EDIT: According to Tempur-Pedic’s current size chart for this pillow (I’m adding a photo of it from their site), you’re supposed to measure the distance between your shoulder and the side of your neck to figure out which size you need. But according to their chart, I’m supposed to use the medium pillow—which is too high for me. So take that measurement, but also measure the length of your neck. Match your neck length to pillow heights, and that may give you a better idea of your best pillow size. END EDIT I wish this pillow had a slight curve along the edge to provide a little shoulder room. Earlier this year I tried Tempur-Pedic's Contour Side-to-Back pillow, which does have a curve for shoulders, but that thing's ridiculously hard and I could never side-sleep on it without serious ear pain. This small neck pillow works fine without the curve. It'd just be kinda nice to have. I also wish Tempur-Pedic would make contoured pillowcases for these neck pillows, especially cases made of silk or polyester satin—something that won't destroy my hair overnight. If you're buying the small pillow, try a toddler pillowcase. Be sure to check measurements, of course. UPDATE: I found another silk pillowcase that fits this pillow perfectly! ( LilySilk pillowcase, 19 momme mulberry silk, 15x22 inch, envelope closure ) I'm surprised this pillow's so lightweight. The side-to-back pillow I mentioned earlier was so hard and heavy, I could probably knock someone out cold if I swung it at 'em. But I could probably put this small neck pillow in my suitcase, and I'll probably start doing that whenever I travel. (I know Tempur-Pedic makes a travel sized neck pillow, but according to their measurements it'd be too high for my neck, just like the medium sized pillow.) Oh, one more thing: yes, this pillow does smell like a chemical factory upon delivery. Not good if you have a very sensitive nose, or if you're migraine prone like I am. I simply propped mine vertically on its side, in a well ventilated area of my home, and let it air out for about a week before using it. The pillow still has a slight smell, but it's not nearly as strong anymore. I could probably spritz a little lavender linen water on the pillow if the smell really bothered me. But so far, it hasn't triggered a migraine or sinus headache—which is more than I can say for some people's perfume/cologne. Still, it's definitely something to be aware of if you're very sensitive to smell.
